Twin pregnancies 
special examinations

The first trimester ultrasound examination creates an opportunity to distinguish between the two basic forms of twin pregnancies. In a dichorionic twin pregnancy, two fetuses and two placentas develop in the pregnant woman's womb, in a monochorionic pregnancy, two fetuses share one placenta.
In the case of both monochorionic and dichorionic pregnancies, during the correct ultrasound examination, the chromosome and developmental disorders can be screened, but these are performed using different methods than in the case of some pregnancies.

In the case of monochorionic twins, the number of possible complications during pregnancy is significantly greater than in the case of a dichorionic pregnancy:

  • Twin-Twin Transfusion States - TTTS
  • Selective fetal retardation/discordant growth - sIUGR
  • Twin-anemia polycythemia sequence –TAPS
  • Acardicus twin - TRAP
  • Death of one fetus
  • Higher incidence of developmental disorders
  • Higher incidence of cardiac malformations
  • More frequent occurrence of premature birth

It follows from the above that the classification of the given twin pregnancy into the mono- or dichorionic group is essential. The separation of the two types is based on a number of ultrasound features (number of placentas and their position, thickness of the separating membrane, adhesion: twin peak - lambda signal and T signal), the recognition of which requires great expertise.

Based on the diploma issued by the FMF, we are the only one in Hungary to have gained international expertise in the care of monochorionic pregnancies. In addition, we have participated in nearly 200 intrauterine fetal operations in the treatment of twin-twin transfusion conditions. Timely recognition of the previously mentioned possible complications can significantly improve the favorable outcome of monochorionic twin pregnancies.

In addition to the above, it is important to know that prenatal care in monochorionic and dichorionic twin pregnancies is fundamentally different from individual pregnancies. Our goal is close maternal and fetal monitoring, timely recognition and treatment of complications (preventing the birth of premature babies and preventing the development of maternal conditions such as preeclampsia, HELLP syndrome, gestational diabetes, etc.).
